P4310 and Taxes

Has anyone seen PO entry (P4310) retrieve the wrong tax explanation code and tax rate/area in the PO header?

The user has entered in several PO. The header has the wrong tax explanation code and tax rate/area. It's an intermittent problem. The processing option is pointing to the supplier address book information for the tax info.

If someone has seen this, then what was the resolution?
