The scenario is that we have a list of customers that would get a free item with their next order can we handle this without having to reload the customer groups so that a given customer does not get the free goods more than once

One way you might be able to accomplish this is to enter a Sales Quote for each of the customers on your list for the free item. Then you can pull these items onto a Sales Order and once released from the Quote, the Sales Quote would no longer have a quantity available.

I haven't tested this out, but in theory should work.
