P4205: Commit Failure error

While using this application P4205, we have kept transaction processing ON. The transaction is being rolled back and records remain in F41021WF. Can anyone please help me with the possible reasons of the transaction getting rolled back? Also how can we prevent it? Help..!!! Don't know much about this application.

Turn on debug logging and look for primary key violations and other sql update errors.

Yes..I checked the logs..the following error occurs multiple times in the log:

[MAFLETCONTAINER] ERROR: unable to get node by ID (451_.._4512443) because it wasn't in the NodeDataSource individual node cache.

Didn't get much info about this error. Any idea on why this error occurs?
