Generate PO and line 1 missing

Twice within the last week, when a purchase order is generated from a requisition, line 1 does not come over yet the req. says line 1 is closed. I can not reproduce the problem. Has anyone else heard of this? Thanks in advance for any input

I have more information on this. I found the missing line 1's in the F43199 with a zero document number. This user has intermittent database connectivity problems, but that shouldn't trash the database. Does anyone know of any SARS to fix the document number of zero being assigned?
