Insert New task in Solution Explorer

I am trying to insert a new task to an existing Task (Parent Task). I did right click on a parent task and then selected insert new task and made all the appropriate entries in the form and click OK. To my surprise I cannot see the newly added task as a child task.

However the task itself has been created in F9000 table but there is no entry in F9001 (Task relationships) table.

Am I doing something wrong? Any suggetions / help will be appreciated.

I have noticed this from time to time (have not located the root cause); in those cases, I right click again, select Task Relationships, and manually connect the Parent and Child tasks.
