Why the F4209 it's not recorder sometimes after a SO is released from a Hold Code?

We have two cases where the Hold Codes are not appearing in the order but they are on the table F4209.
Moreover, on that table there's no record from whom and when release the Hold Codes so the order is now without a HC and we don't know who did it.

Can anyone help me please?

One of the reasons we create audit records on the F4201 and F4209 is to determine who/when/how an order went on hold and was subsequently released. See E1: CFR11: Auditing Tool (21CFR Part 11) Guides and MTRs (Doc ID 636234.1) on My Oracle Support for information on how to set this up. While it won't help you with prior orders, it could be helpful for future orders.
