Harold,
We had similar problems once and after reviwed all the things that you just
mentioned, we start to see the printer definitions and guess what? it was a
paper definition on the printer, I mean the physical printer. What kind are
you using?? for example Epson has some "special" configuration...good luck

Harold,
We had similar problems with our Epson LQ-2170. After talking to epson people we changed the driver for the printer to an Epson LQ-1070 Scalable Font. This solved our problems so it may just be that your print driver doesn't understand what is being sent to it.
