Help with creating a report - Item Adress book

Hi Please help me create a report for the following columns in my report Address number, alpha name, area code, phone number, mailing name, city, state, postal code and country.

Need help with the code part also

F0101 will be having your address book number & alpha name.
F0115 will have phone details
F0116 will have Address details.

F0101 and F0116, F0115 is one to many relationship.

Design:
1) Main section on F0101
2) Custom section on F0115 and F0116
