CRM Lead application P90CB010 is not creating an opportunity

In JDE E1 9.1 in CRM module the lead application P90CB010 is not converting in to an opportunity. Created a lead and after saving when clicked on the convert lead button, the application is not processing. Neither a customer is created nor an opportunity. Request anyone to provide resolution for this. All the required set ups are done.

I had the same problem when I attempted to convert the lead into opportunity. Did you manage to make the proper setups for this application?
